Saw this clip in the deadlock discord, match ID 71471191. In the clip you can see the enemy team ends AM's first phase, and then their minions do the same to the HK. However rather than pushing both into their second phase, the game ends prematurely and gives the win to the dead team entirely skipping the second patron phase on the HK.
This happened to me a month ago (though, from the side that was winning and lost) and had pretty much the exact same scenario. We were in their base, we defeated AM's first phase, and then when our HK's first phase was defeated by minions we instantly lost without the HK entering the second phase. The match ID for this match is 61544176, though this was in february so the replay is not viewable anymore, and I didn't know replays existed at the time so I don't have a recording of it
