I saw this being discussed in the discord and I remembered it happening in my games so I thought I'd make a thread to give some details which could help
I've seen it happen a few times with Leech specifically, not sure if I've ever seen it happen with any other item
from what I remember, the duplicate usually shows in a flex slot, and I don't think the slot has to be unlocked for it to show there
sometimes it'll show everyone on the other team (maybe both teams actually) as having the item when they don't
if I remember right, it's visible to everyone when it happens (I've pointed it out to friends who could see it too)
and it usually goes away after a minute or so
