Specifically, you need to ult at an angle that hits the top of the box on the corner otherwise the projectile will pass through. The box breaks, the game considers it a miss so it refunds half your ult cooldown, and then it bounces to an enemy hero still dealing damage. Additionally, it seems to also spawn a bugged ult projectile that sticks to the enemy hero but does not deal damage.
