Pretty easy to reproduce - both abilities and items. Direct experience with healing rite and viscous cube. Walk face first into an object or wall and try to self cast the item/ability, doesn't work. Back away from the cover just a bit, and you can cast. I've died a few times from this when trying to cast my cube while hugging cover.
