What I assume is happening, If you animation cancel in certain way (I can't remember), the game makes Ava the default character and you are stuck like this for the rest of the game, although I've only tried in sandbox. Even death or recasting Ava doesnt fix this. Being Ava is a disadvantage too, as you cant dash on the ground , your height severely messed up, and your gun is effectively in the ground.
