Concept is simple
Tier 3 and it is standalone
You get a passive that allows you to debuff enemies hit (bullet hits reduce spirit res. spirit hits reduce bullet res.)
Catch is that you also debuff yourself for 1.5x amount, and debuff linger only on last target hit for around 3 seconds since proc.
Item should be green, giving little bit of both resistances(10% should be good) and chunk of hitpoints like fortitude (200hp+)
Debuff itself should be 20-30% resulting in cutting 30-45% of your resistance in return.
If target is killed you receive a healing buff of some damage target did to you, but resistance debuff still linger for duration of healing.
So you activate - focus target - if target is offed you dip, since you need to recuperate and not die via heal is active, if target not killed you received a lot of damage, lost more of your resistances for duration of passive proc than your target, which makes continuing fight a bad idea.
Item will require a lot of other items to make it work since it doesnt offer offensive capabilities, but in the end good team fight item or dueling utility(like hunter aura).
