Yeah that's quite a nice item, it's sort of a counter to movement slows that I personally wish there was an item for that.
With the recent movement nerfs I don't expect a tier 4 item like this to be too problematic.
As for criticism, I would definitely get rid of the movement debuffs the item gives since I think they are way too debilitating for such an expensive item that is meant to help with escapes.
3m/s as a buff is not even that amazing, basically comparable to fleetfoot while also quite a sizeable amount of resist.
Remove the movement nerfs and just lower the weapon damage and resist you get and the item should be good.

It could be a tier 3 actually, probably unchanged. I think it would be better that way.
