When placing a bomb on top of an enemy, the audio que will sound, however the ability isn't casted. I think the ability acts like a cast and not an instant cast, for if when you cast the ability the enemy rolls out of the area of range within a second the bomb won't be placed.
The Ultimate ability has an aoe effect but it seems to be canceled by line of sight, its not like Mcginnis' ulstimate where the aoe is not affected by line of sight.
 
Yep, I just recently posted about the bomb in the gameplay feedback too. It's frustrating when you think you stuck a bomb, then you echo shard, but then realize the first bomb was never placed.

For now I've seen builds recommend mystic reach to deal with this bug

Why would that help? The issue happens at the edges, so extending the range only moves the edge. I often build improved reach onto bombs and still encounter this behavior.

Couldn't tell ya. That's what the build I'm using says works.
