this happened to me as well on Abram, died mid animation while casting my ultimate and respawned like that. I did go back and check the replay and it looks normal,so its just a visual thing during the game
The same thing happened to me and to a friend I was playing with (match id: 86415. Happened at 31:20). It doesn't seem to be exclusive to Abrams because it happened with Kelvin and Bebop in the match I was playing.
I cant upload the video but I encountered the same issue. It seems to have happened when I pressed show replay on death then respawned close together. Animation was stuck on when I started the replay.
