Usually after buying a third active item the icons bug out showing I only have one active item one that I hadn't bought, it also shows them in the wrong spaces at times. After using the active items that it shows that I don't have they pop up in the right spot again and the bug doesn't happen after I have used the active items I have. It happens just about every game without fail after the latest big patch.
