Soul Exchange "swaps health levels" and isn't affected by bullet or spirit resist. Shiv's passive counts the Soul Exchange health loss as damage taken, and thus lets him defer the health loss as though he had taken damage. Given the wording on Bloodletting, it seems like an unintended interaction.

Hopping on this to elaborate: I want Bepop's #2 to be hold to alternate cast, but I want every other hero's alternate casts to be double tap. Assigning Bepop Specific Hotkeys doesn't work as expected, if I have Bepop set to hold, then every other hero is also set to hold as well as the "Default"...
