Dynamo's ult VFX disappeared after love bites and spirit burn were procced. (Can't seem to recreate it in testing however).
Yes, it would always upgrade like normal. But I just saw another person with the same issue as you so it seems it only happens to some people, or there is something else needed to cause it.
I was referring to the latter part of your post "ALT only works if you use the default 1, 2, 3 and 4 hotkeys. If you use a different hotkey (mine are M4, M5, E and F) the game doesn't upgrade it"
There are dedicated upgrade binds which are alt + number. If you change the ability key to E for...
Holding the control key while holding alt to upgrade abilities uses the ability instead of upgrading it. (And because crouch is control by default, this happens to be sometimes in real games).
The current behavior is how it already worked before that patch note, so I would say no, it is not intended. By actual distance, the patch note means distance the bullet travels, not the physical distance without the door.
November 21 patch stated that "Bullet damage falloff and effectiveness now use the actual distance traveled, rather than the distance between the starting position and end position" but this is not currently true.

I use Majestic Leap as soon as Ethereal Shift ended naturally. I get a barrier and it goes on full cooldown but I dont move at all.

I used ground strike and mantled at the same time. Ground strike went on cooldown and the VFX and SFX from using it persisted.
