Came across this issue when playing as Rem.
I got the Signet to show up in my picks. I selected to replace my Rapid Recharge with it and the screen just locked up. Even more oddly, though, not only did I not get the Signet, but Rapid Recharge then got downgraded back down to Extra Charge...
