It's actually some thrown projectile. Ivy bomb, alch fire flask, Haze dagger, Vindicta 1st and 3rd, Paradox bomb, Kelvin bomb. Those are all the ones I've found.

Different items, such as Melee Charge, doesn't fix it or makes it better. Seems to also not happen 100% of the time, but at least 80%.
