The same thing happens to me (AZ) and a friend (GA). I haven't ruled out that our ISP's are to blame though.

During my most recent match (ID: 31920135), alchemical fire would not come out on command. At some point, there was a bug where after respawning, alchemical fire would be auto-equipped, but my character (Paradox) would not raise her hand. I couldn't get rid of it other than throwing it in base...
