My friend has the exact same problem. He tries to use all these performance enhancing settings and mods but the visual glitch always happens

I think i got disarmed at somepoint in the game and it just never went away even after dying, couldn't shoot all game.

Test Hero while I was joining a match was active and I joined the test hero match while joining the match and it said the test hero gamemode was a real match and gave me a penalty for leaving test hero mode.
