Crushing fist seems to only give back ammo when there is already ammo in the gun. If you are at 0, it just starts a regular reload. Crushing fists only will then give you ammo if you shoot again and end up with ammo in the gun.

We had 5 players crash at the same time. A couple people crashed later in the game as well but I don't know if both events were related.
