Haze stuck in ult animation

Haze stuck in ult just to me, visual and audio cues. Occured on first ult cast, Did not reset upon death and kept going on her model in spawn. Match ID: 12643

Same thing happened to me. I wish I could remember which game it was, but I was in a game where Haze was stuck in her ultimate animation all game. My friend was in the game and he did not see her stuck in that animation.
