FPS looks high, but seems to be running super low (100+ will look like 3/4 frames per second)

It feels like a different issue than people getting outright low frame drops. I'm on a 4080 super, 13700k, 32gb of ram, etc (dxdiag is attached). I'm not getting this issue in other games, so I'm not sure whats going on. I'm going to try closing a bunch of processes and uninstall some asus/manufacturer bloatware and see if that helps, but this doesn't happen in other source 2 games either.

I'm also running the game on max settings (with FSR so "max) and on vulkan, I switched back to DX11 and it seemed to be running normally.

I used to average around a steady 120 fps, but now I usually drop to 90-40 end game. Usually high fps in early game.
